Carla Abruzzo: Contract of Agreement for Freelance Makeup Artistry

This contract stipulates that Carla Abruzzo, will provide makeup services for:

I, Carla Abruzzo, will arrive at (time) at the following location:

I will perform my duties as your artist for (any event on price list)

That will result in the price of (dollar amount negotiated)

Travel

I am willing to travel, but there will be extra cost for anything over 20 miles out of range. If there is parking costs that will also be added to the final price. You will be given a receipt at the time of service.

Payment

I do not ask for deposits, in return I do expect full payment on arrival.

Artist will be paid by check or cash for the services rendered and travel expense.

I want this to be an enjoyable experience and not burden, so I have no cancellation
fees, just expect a 72 hour notice!
